ADHD Medication Shortage Alert

There is an ongoing severe shortage of controlled stimulant ADHD medications occurring nationwide.  We estimate this shortage will prevail for some time.

Student Health Services (SHS) pharmacy is at a critical point with ADHD medications where inventory is not enough to meet the demands of UCF students.  SHS receives a minimal sporadic supply of these medications, if at all, and our pharmacy is subjected to daily, weekly, and monthly quotas of controlled ADHD medications.

Health Information Management and Compliance

The HIM department staff is responsible for the quality control of patients’ health information and the handling of all requests to release or obtain health information. The HIM department is located on the 2nd floor of the UCF Health Center.

Medical Records

The best and quickest way to view your immunization record, lab results, and summary of visit is to log into your Patient Portal account.

*IMPORTANT* If you do not receive an e-mail copy of your submission to the e-mail address you provided, your submission DID NOT go through.

Please note:  To authorize sensitive information (including but not limited to HIV/AIDS, STD, psychiatric records, drug and/or alcohol abuse, and sexual assault records) to be released, you must initial when prompted on the form.

Patients who are a minor (17 years old or younger) at the time of completing the form, will need a parent or legal guardian to sign the form.

Records for Personal Use Only: there may be copying fees incurred:

Portal/Email (more than 5 pages) $6.00
Portal/Email (less than 5 pages) No charge
Mail/Paper $1.00/page for the first 25 pages

$0.25/page for additional pages

There is no fee for sending records to another facility.

Consent to Discuss

To authorize UCF SHS to discuss your medical care, treatment, billing, services, etc. to a designee, please submit a Consent to Discuss form.

**NOTE** The Consent to Discuss does not grant permission for the following actions to the designee listed below:

  • scheduling appointments on behalf of a student (18 or older)
  • releasing copies of medical records i.e. (billing statements, laboratory results, or treatment services, etc.)

Consent to Treat a Minor

For patients under 18 years of age parental consent is needed for treatment. Parents may consent by signing Section D of the required UCF Mandatory Immunizations

All UCF SHS staff abide by the Health Insurance Portability and Accountability Act (HIPAA) enacted to protect the privacy of patient health records.If at any time you feel your privacy rights have been violated, a written complaint may be submitted to the UCF SHS HIPAA Privacy Officer or to the Director of Office for Civil Rights (OCR) at U.S. Department of Health and Human Services.
