Immunizations


New Student Requirements

Prior to registration, all new students accepted for admission must submit UCF Mandatory Immunizations form. All students born after December 31st, 1956, must provide proof of immunizations.

WHAT IS REQUIRED:

  • Proof of TWO (2) doses of measles and ONE (1) dose of rubella given AFTER your first birthday with the 2nd dose given at least 30 days after the first dose (commonly given as a combined vaccine called the “MMR”).
    • Lab results (IgG titers) showing proof of positive immunity to measles and rubella may be submitted in place of the two-dose MMR requirement. An equivocal or negative titer will require the student to receive the MMR series.
  • Proof of three (3) doses of the hepatitis B vaccine series. If you do not have proof, you can sign and date the waiver in section A. If you are under 18, your parent/legal guardian will have to sign and date the waiver.
  • Proof of the meningitis vaccine AFTER the age of 16. If you do not have proof, you can sign and date the waiver in section A. If you are under 18, your parent/legal guardian will have to sign and date the waiver.
  • Are you currently under the age of 18? Please see SECTION D on the health form. Your parent or legal guardian is required to sign the form and indicate their relationship to you, and date this section.

Supporting documentation accompanied with the UCF Immunization Health History form is also accepted (no doctor’s signature and stamp are needed in this case). If you have a vaccine record with your name on it and the vaccines meet the requirements, we can accept this as proof. For example, a child’s vaccination card, records from a school or another university, a physician’s office or state health department, etc.

Submit Documentation to Med+Proctor

UCF has partnered with Med+Proctor for immunization compliance and verification. No forms will be accepted via email due to health information privacy laws. You will not be able to register for classes without completing this requirement. The Immunizations hold on your UCF account will be lifted automatically within an hour AFTER you receive a compliance confirmation email from Med+Proctor. We recommend starting this process early so that you will be able to register for classes during your orientation session.

NOTE: Med+Proctor is a FREE service and the $10 fee is NOT REQUIRED. If you would prefer to expedite your record processing time, then the $10 fee would apply. Otherwise the standard processing time (2-3 days) requires no fee.

Exemptions

Military Exemptions

Students who are active-duty or veterans may submit the UCF Immunization Health History form with waivers (under section A) signed and a copy of their military ID or DD214 form.

UCF Online Only Student Exemptions

Students who are enrolled in an Online-Only program are required to submit the UCF Immunization Health History form with waivers (under section A) signed.

Medical Exemptions

For students with certain medical conditions and/or contraindications to the MMR vaccine, please contact our office by calling 407-823-2701 (opt. 4) or emailing immunizations@ucf.edu for necessary documentation

 

Vaccine Information

Measles

Measles (Rubeola) is a highly contagious viral infection and can cause serious health complications like pneumonia and encephalitis, brain damage, or even death. RUBELLA (German Measles) is a contagious viral infection that mostly presents with mild symptoms (low-grade fever, sore throat, rash, stiff joints). However, for women, it can cause a miscarriage or serious birth defects in a developing baby. The best prevention is a two-dose measles-mumps-rubella (MMR) vaccine given after the first birthday.

Per CDC guidelines, students at post-high school educational institutions without evidence of measles immunity need two doses of MMR vaccine.

Hepatitis B

Hepatitis B (HBV) is a liver infection caused by the hepatitis B virus. It can be a short-term illness with mild to severe symptoms or a life-long infection with serious health problems (liver damage, cirrhosis, liver cancer, and even death). The Hepatitis B vaccine is available to all age groups to prevent the Hepatitis B viral infection.

Waiver Statement-Hepatitis B: Missed doses may still be sought to complete the series if only one or two have been received. The HBV vaccine has a record of safety and is believed to confer lifelong immunity in most cases. Signing a waiver indicates that you understand the possible risk involved in not receiving this immunization. If you are under the age of 18, a parent or guardian must sign the waiver for you.

Meningococcal Meningitis

Meningococcal disease is a rare bacterial infection of the membranes surrounding the brain, spinal cord, and bloodstream. It can cause severe neurological damage, loss of limbs, or even death. The groups most at risk are adolescents and young adults aged 16 through 23 years old, and people with certain medical conditions that affect the immune system. FDA-approved vaccines are currently available that decrease a person’s risk of acquiring meningococcal meningitis.

Waiver Statement -Meningococcal Meningitis: Vaccination is one of the most effective ways to prevent meningococcal disease. Signing a waiver indicates that you understand the possible risk involved in not receiving this immunization. If you are under the age of 18, a parent or guardian must sign the waiver for you.
